01 · The business

A flat admin fee, not a cut of drug spend.

Rightway runs pharmacy benefits for self-funded employers and sells care navigation. Under its SureSpend model it earns an administration fee rather than revenue from drugs dispensed, passes 100% of rebates to the plan, owns no pharmacies, and refunds every dollar of spend above a set monthly cost, with no cap. It does not publish fees or revenue.

The moat, in three parts
  1. 1
A model the big three are being pushed toward
Rightway already passes all rebates through and owns no pharmacies. Congress and FTC settlements are now pushing CVS Caremark and Express Scripts toward similar options.
Rightway, Jan 2026; AJMC, Jul 2026
  2. 2
Pharmacists and nurses inside the benefit
Remote clinical pharmacists act as a 'pharmacist in the family', and nurses handle triage and navigation, which most fee-only PBMs do not bundle.

  3. 3
Proof it can run very large plans
Tyson Foods, with about $200M of pharmacy spend and 140,000 staff, moved from CVS Caremark in 2024. Rightway now counts 45 Fortune 500 clients.

Francisco Partners led the September 2026 Series E. A Bloomberg-linked report puts the valuation at $1.75B, citing a person familiar with it.

An up round in a sector under political fire. The paper value of 2021-era options likely rose, though the reported price is not confirmed.

    Good

Congress wrote its model into law.

The Consolidated Appropriations Act, 2026, signed Feb 3, requires PBMs to pass 100% of rebates to employer plans and report pay and spread pricing, from plan years starting about Jan 2029.

Rightway already works this way, so the cost of compliance should be low. But its main selling point becomes a legal minimum.

    Watch

Big rivals must now offer pass-through.

FTC insulin settlements made Express Scripts (Feb 2026) and CVS Caremark (Jul 2026) offer options like point-of-sale rebates and exits from spread pricing. Optum Rx was still in litigation in July.

The giants can now pitch a cleaner product to the same employers Rightway targets.
